Level
II - Higher Paths
IntraCare
Adult Day Treatment Program offers two levels of care; Partial Hospitalization
Program (PHP), which is a 5 day a week program and Intensive Outpatient
Program (IOP), a three day a week program. These two programs are
designed to meet the Mental Health and/or Chemical Dependency treatment
needs of patients who are at a higher level of functioning. The
PHP program runs Monday – Friday from 9:30 a.m. until 3:00
p.m. The IOP program operates within the framework of the PHP; however,
the days of attendance are negotiated with the patient to best meet
their recovery needs.
A
Primary Therapist will be assigned to the patient. The therapist
will work closely with the patient to address their issues, and
with their permission, will also work closely with his/her family
in family therapy and discharge planning.
Several
treatment modalities are offered in PHP. These include, but are
not limited to: Group Therapy, Individual Therapy, Family Therapy
and Nursing Groups. Step Groups, Addiction Education, and Relapse
Prevention groups are also provided.
Patients
are provided individualized treatment within a team framework. Members
of the treatment team include the patient, the patient’s doctor,
nurse, program therapists, Licensed Chemical Dependency Counselors,
and psychiatric technicians. The patient will be given a schedule
of his/her treatment modalities. This schedule will fit the initial
needs of the patient, but may be modified as the needs of the patient
change.
Lunch
is provided for patients free of charge. IntraCare Hospital provides
a safe, structured environment to allow the patient the opportunity
to take risks, explore and develop positive coping skills, to enhance
a sense of personal identity, and to increase the patient’s
self-esteem and personal values. The Higher Paths program focuses
on cognitive therapies and higher level processing. Actively working
a recovery program is the essence of treatment in IntraCare Hospital’s
day treatment programs.

Level I -
Foundations Program
The
IntraCare Foundations Adult Day Treatment Program offers two levels
of care; Partial Hospitalization Program (PHP), which is a 5 day
a week program; and Intensive Outpatient Program (IOP), a three
day a week program. These two programs are designed to meet the
Mental Health and/or Chemical Dependency needs for patients who
may need to work at a slower pace. The PHP program runs Monday –
Friday from 9:30 a.m. until 2:45 p.m. The IOP program operates within
the framework of the PHP; however, the days of attendance are, negotiated
with the patient to best meet their recovery needs.
Transportation
may be provided to seniors (over age 60) and individuals who reside
in assisted living facilities or personal care homes within a 10
mile radius of IntraCare Hospital. Individuals with Medicaid insurance
may be eligible for transportation through the Medicaid program.

Upon arrival, the patients meet the treatment staff that consists of a RN, therapist/case manager, LCDC, and psychiatric technician. The patients see their physician at least once weekly while in PHP.
The
patient begins the day with a group that starts at 9:30 (Morning
Focus). The patient’s day is highly structured with groups
at 10:30, lunch at 11:30, group at 12:30, and group at 1:45. The
patient’s day ends at 2:45. Groups are primarily educational,
experiential, and didactic with such topics as stress management,
discharge/weekend planning, loss resolution, life skills, relapse
prevention, medication education, management of my illness, etc.
Group work is designed to allow patients to work at a slower pace
as needed in order to acquire the basic skills needed to process
and assimilate new information and utilize their new skills.
Individual
and family sessions are offered weekly. As much as the patient is
willing to allow, family members are included to assist with discharge
planning.
